Please be advised that all patients will be charged according to our cash rate at your appointment visit and it is solely your responsibility as the patient to submit required documents to your insurance for out-of-network reimbursement. All required documents will be provided by our office at your request. It is important to be aware that Blue Shield of California has recently implemented policy changes that significantly limit the amount of reimbursement for Chiropractic Services.
Therefore, if you intend to utilize this insurance, it is highly likely that it will not be applicable in our office. For individuals without insurance, choosing not to use insurance, or those participating in an HMO plan, you will be considered a "Cash Patient."
