All plans do not include your initial evaluation and treatment visit. I understand by choosing a payment plan that I agree to make the selected total cost payment in full at the time of selection. I understand that it is my responsibility to make such payment and that this is my contractual agreement with Dr. Jimmy Sayegh for my care. I understand that all pricing listed above is reserved for prompt payment plans ONLY and cannot be applied visits outside of this plan. All treatment plans are non-refundable and non-transferable to another person. Treatment plan pricing does not apply to personal injury cases. No previous treatment visits can be applied. Cost of treatment cannot be applied towards in-office purchase of products, appliances, or home care devices. Any appointments that are scheduled and missed or canceled within 24-hours of scheduled appointment will be subject to a one visit deduction from total visits. Visits only qualify for weekdays (Monday – Friday), weekends or special events are not included. If you elect a sport therapy option, please make sure to schedule in advance as we cannot guarantee same-day accommodations.
